Funding Forms
Updated March 1, 2006Below is a collection of links to forms and resources that could be helpful in conducting your research. They are current as of the above date. Most forms are either downloadable in both Microsoft Word and Adobe PDF format. Most of the PDF forms have the capability to be filled out on the computer. However, note that you will not be able to save these PDF forms as a filled form unless you have the full Adobe Acrobat application. Acrobat Reader will let you open and fill the form but will not allow you to electronically save the filled version. (Adobe Acrobat Reader for PDF is available

Institutional Review BoardAll research involving human subjects that is conducted at or sponsored by the University of Illinois at Urbana-Champaign, whether funded or unfunded and whether conducted by UIUC faculty or others, must comply with applicable policies for the protection of human subjects. The IRB Office, while performing administrative functions of the IRB, also serves as the official oversight office for human subjects research.
IRB-1 ApplicationAll human subjects research projects must be reviewed and approved before human subjects are recruited or involved in research activities. All projects, regardless of exemption status or the type of review they are given, must be presented on an IRB.

School Research FormThe Bureau of Education Research also has a number forms and applications for conducting research in elementary and secondary schools. This form must be used for any research project that will take place in an elementary, middle, or high school setting.
